Rehab hospital listed among nation’s tops for low readmission rates

Rehabilitation Hospital of Southern New Mexico (RHSNM) has been ranked 10th among inpatient rehabilitation hospitals nationwide for lowest readmission rates, the hospital said in a news release.

Becker’s Hospital Review released the ranking based on data provided to the Centers for Medicare and Medicaid Services, RHSNM said. The data measures patient care provided by inpatient rehabilitation providers, including hospital readmissions within 30 days of discharge.

“Without a doubt, we were able to achieve this amazing recognition because of our healthcare team’s knowledge, skills, and commitment,” RHSNM CEO Sabrina Martin said. “Our physicians and employees are passionate about achieving the best possible outcomes for our patients, and that’s reflected in this ranking. To be identified as a leader among our healthcare colleagues nationally is an honorable distinction.”

RHSNM provides specialized rehabilitative care to patients who are recovering from disabilities caused by injuries, illnesses or chronic medical conditions, the news release said. The hospital’s services include physical, occupational and speech therapy; rehabilitative nursing; respiratory therapy; dietary services; case management; pharmacy services; social work services; pain management; and more.

The hospital also has a Transitional Outcomes Program designed to increase patient safety and decrease hospital readmissions. Patients are followed for a minimum of 30 days after leaving the rehabilitation hospital to ensure effective and appropriate re-integration into the community.

RHSNM, 4441 E. Lohman Ave., treats about 900 patients annually, the hospital said.
